Cortez Training Center (CTC) is a Black Owned, Veteran Owned, Limited Liability 501(c)3 Non-Profit Organization, and Florida State E60 National Taxonomy Exempt Entity (NTEE) that has been acting in its mission since 2019 and incorporated since March 2nd 2021. This corporation is a Health Support Organization that provides equipment, services, community, resources, and other forms of support which are complementary and integrative approaches to prevention, treatment, or rehabilitation of dysfunction, illnesses, injuries, or disabilities. CTC uses an inter-professional core of health support services such as personal training & nutrition coaching, yoga, pilates, corrective and functional fitness, physical therapy, occupational therapy, massage w/Rolfing, acupuncture, reiki therapies, and more.

Donation Policy: Cortez Training Center Non-Profit accepts tax-deductible donations under 501(c)3, NTEE E60. Our standard donation policy follows the privacy policies listed below with donor information.

Privacy Policy

Cortez Training Center respects the privacy of client information and follows strict confidentiality standards.


Protection of Personal Health Information (PHI) and Personally Identifiable Information (PII) 


At Cortez Training Center (CTC), safeguarding the confidentiality and security of Personal Health Information (PHI) and Personally Identifiable Information (PII) is of utmost importance. CTC uses Jotform survey applications with HIPPA Compliance, and Salesforce platform to maintain privacy and security within legal regulation. The following policies are in place to ensure compliance with legal requirements and protect the privacy of individuals associated with CTC:

Client Media Consent: Clients acknowledge and consent to being included in photographs, videos, or other recorded media. Such media may be used for promotional or marketing material and/or placed on the organization's website or social media platforms, unless explicitly stated otherwise. In cases where explicit consent is not obtained, the organization ensures compliance with privacy laws and regulations.

Confidentiality Agreement: All staff, volunteers, and individuals handling PHI or PII must sign a confidentiality agreement, affirming their commitment to maintaining privacy and security.

Limited Access: Access to PHI and PII is restricted to individuals with specific roles requiring such access for the purpose of health support services. Unauthorized access is strictly prohibited.

Secure Storage: Physical and electronic records containing PHI and PII are securely stored. Electronic records are protected with encryption and access controls.

Training: Regular training sessions are conducted to educate staff and volunteers on the proper handling, storage, and transmission of PHI and PII. Training includes legal obligations related to privacy and data protection.

Legal Compliance: Cortez Training Center adheres to all relevant laws and regulations governing the handling of PHI and PII, including but not limited to the Health Insurance Portability and Accountability Act (HIPAA) and other applicable data protection laws.


Data Security Measures: 

CTC implements robust measures to ensure the security of data, including PHI and PII: 


Firewall and Antivirus Protection: All systems handling data, including PHI and PII, are equipped with firewall protection and regularly updated antivirus software. 


Regular Audits: Routine audits of information systems are conducted to identify and address vulnerabilities or security breaches promptly. 


Secure Communication: Encryption protocols are employed to secure communication channels, especially when transmitting sensitive information. 


Incident Response Plan: CTC has an incident response plan to effectively address and mitigate the impact of security incidents or data breaches, ensuring compliance with legal reporting requirements.
